“Vomit” means both the act of throwing up and the material that is expelled. In BDSM and fetish contexts, vomit play refers to sexual or power-dynamic scenarios in which vomiting, disgust, humiliation, or taboo-breaking play a role. Colloquial terms may include puke play, dirty games, disgust play, or “Roman shower.” These terms are not used consistently and may mean different things depending on the community or individual.

Vomit play is a particularly intimate and high-risk practice that should occur only with explicit, informed consent from everyone involved. Vomiting can be triggered by stress, a gag reflex, alcohol, illness, eating disorders, or other strain, and it is not medically trivial. Risks include choking or aspiration of vomit, irritation of the esophagus and mouth, fluid and electrolyte loss, circulatory problems, and transmission of pathogens. Vomiting should not be induced casually, under pressure, or as a dare.

Extra caution is required in cases of pregnancy, gastrointestinal illness, reflux, cardiovascular problems, respiratory conditions, eating disorders, substance use, or strong psychological disgust or panic responses. The mouth, eyes, mucous membranes, and open skin should be protected from contact; hygiene, stop signals, sober decision-making, and aftercare are essential. Shortness of breath, blood in vomit, persistent vomiting, severe weakness, confusion, chest pain, or collapse require medical attention.
